Nattokinase plays a supporting role in heart. Nattokinase is a fibrinolytic enzyme from fermented soybeans (natto). A 2023 meta-analysis of 6 RCTs found significant reductions in systolic BP (-3.45 mmHg) and diastolic BP (-2.32 mmHg).
What dose to look for
Clinical studies typically use 2000–4000 fu of nattokinase. 2,000-4,000 FU/day for cardiovascular support. Meta-analysis of 6 RCTs (546 subjects) showed significant BP reduction at 2,000 FU. Higher doses (10,800 FU) may be needed for lipid effects. Products below this range may not deliver meaningful results.

LOW — Promising fibrinolytic properties but limited clinical outcome data · 2,000–4,000 FU/day (100–200 mg)
- •2017 RCT (82 patients) found 6,000 FU/day nattokinase for 26 weeks significantly reduced systolic and diastolic blood pressure. Mechanism may involve ACE inhibition and fibrinolysis. PubMed
- •Demonstrates potent fibrinolytic activity in vitro and in animal models — degrades fibrin clots similarly to plasmin. But no large RCTs have tested clinical endpoints (stroke, MI, DVT prevention).
- •Safety concern: theoretical risk of bleeding, especially when combined with anticoagulants (warfarin, apixaban). No long-term safety data in humans.
