A 2017 meta-analysis (33 RCTs, 7,175 patients) found significant improvements in cough and sore throat vs. placebo. Most effective when taken within 36–48 hours of symptom onset. Key active compound is andrographolide, which has poor oral bioavailability — standardized extracts strongly preferred over crude powder. Not recommended for continuous use >12 weeks. Contraindicated in pregnancy.
