About Pomegranate

Pomegranate (Punica granatum) extract is one of the better-studied polyphenol-rich botanicals. Active matrix is punicalagins and ellagitannins, which the gut microbiome metabolizes into ellagic acid then urolithins (urolithin A is the form with most evidence). IMPORTANT producer-status caveat: only ~30–40% of people are 'high urolithin producers' based on gut microbiome composition — non-producers convert ellagitannins poorly. Strongest evidence is for blood pressure and inflammation: Sahebkar 2017 BP meta-analysis (8 RCTs; PMID 27888156) showed SBP −4.96 mmHg and DBP −2.01 mmHg. Bahari 2023 inflammation meta-analysis (33 RCTs; PMID 37507609) showed reductions in CRP, IL-6, and TNF-α. Lipid evidence is mixed/null — Sahebkar 2016 (PMID 26857863) and Wang 2019 (PMID 31987256) found no significant lipid effects; the 2025 update (37 RCTs; PMID 40216355) found only HDL-C +2.50 mg/dL, no TC/LDL/TG effect. Prostate cancer claim (Pantuck 2006 PSA-doubling-time trial, PMID 16818701) was NOT replicated in the placebo-controlled Pantuck 2015 RCT (PMID 26169045, n=183, NEGATIVE) — many brands still cite the 2006 result without mentioning the 2015 failure. CYP3A4 'grapefruit-style' interaction warnings are based on in-vitro data; clinical PK studies (midazolam) showed no effect. Pomegranate juice is high in sugar — extracts are preferred for diabetics. No established RDA/UL.
